What these questions save you from
Rights never discussed
Usage, duration and territory define the licence. The template asks how the illustrations will be used, so you can price the rights.
A style that is not yours
Asking for references shows early whether they came for your style or want you to copy someone else’s.
Final files in the wrong format
Print, web or animation: the output format changes how you work from the first sketch.
Use it as your client form
